He ensured that she would give fatal medication to Harlan. But because Marta actually administered the correct medicine to Harlan, Harlan wasn’t actually dying.
While some might have guessed Ramson’s involvement, the movie makes it clear that Marta was actually aware of what was going on. And this makes the movie stand out among many thrillers that promise huge reveals and memorable moments.
